Ultimately, diff training regimens work differently for everyone. Try it out, do some experimenting and see what type of program your body responds best to.

sometimes...it depends on whether or not you care about fat gain while bulking...if you are trying to lean bulk then yes, only some may be necessary...but this is harder to do honestly, and it also depends on your specific ability to gain fat and lean muscle mass...long story short, I can't answer your question simply although I will say that if you are an ectomorph, dont do cardio while trying to bulk...
